About the Role

Old Southwest Health and Rehabilitation is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. As a key member of our healthcare staff, you will provide high-quality patient care and support to our residents.

Responsibilities
  • Observe and manage patient health according to individual needs.
  • Provide routine care, ensure comfort, and report any changes in patient health and behavior.
  • Collect, track, maintain, and review patient medical history, charts, and physical condition.
  • Administer medications and injections, provide wound care, perform standard diagnostic procedures, and collect and prepare specimens.
  • Other special projects and duties, as assigned.
Requirements
  • Licensed Practical/Vocational Nurse with current state license.
  • Must have a current/active CPR certification.
  • Excellent technical, assessment and documentation skills.
  • Strong analytical and problem-solving skills.
  • Ability to lift fifty (50) or more pounds. Physical work is a primary part (more than 70%) of job.
  • Able to stand and/or walk for more than four (4) hours per day.
  • Able to bend and/or stoop for more than one (1) hour at a time.
Work Schedule

We offer flexible hours, including days, nights, and weekends. You can choose to work full-time or part-time, and we are willing to work around your schedule.
