Responsibilities and Requirements
  • Provide direct patient care within the scope of the State Nurse Practice Act.
  • Implement the plan of care initiated by the Nurse Manager and Clinical Coordinators.
  • Assess and provide patient/caregiver education and information pertinent to disease process, self-care techniques, health promotion, and disease prevention.
  • Report any changes in patient's condition to the Family/Physician with reports documented in the clinical record and implement physician's new orders.
  • Perform drug regimen review at every visit.
  • Participate in coordination of care reporting the identified needs for other disciplines to the Nurse Manager.
  • Perform medication administration per policy and procedures.
  • Educate patient/caregiver in administering of medication, including infection control.
  • Maintain and order effectively adequate supplies and stores correctly.
  • Follow infection control guidelines, demonstrate appropriate use of PPE, perform hand washing per policy, use correct bag technique, disposal of hazardous waste according to policy, and follow safety regulations.
  • Adhere to agency policies and procedures.
  • Attend mandatory meetings, training sessions, in-services, and team meetings.
  • Meet productivity.
  • Perform other duties as assigned or directed to ensure the smooth operation of the department.
Requirements
  • State of Ohio Licensed Practical Nurse License.
  • CPR certification.
