You will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule. You will also have the opportunity to opt into equipment, instrumentation, administrative, and/or Opt-Tech support services for a fair market value.

To be eligible for a lease agreement, you must have a Doctor of Optometry (OD) from an accredited optometry school, a valid state optometry license, and a General Liability Certificate. You must also have Professional Liability Insurance Certificate and a W-9.
