Clinical Navigator

This position requires a highly skilled and compassionate RN to serve as a navigator for patients and families affected by cancer. The ideal candidate will have a minimum of five years of experience in oncology, with a strong understanding of patient-centered care and the ability to provide emotional support and education to patients and their loved ones.

The RN Nurse Navigator will work closely with multidisciplinary teams to ensure seamless coordination of care, promoting optimal patient outcomes and quality of life. This role involves collaborating with clinical staff, physicians, and community resources to address financial, social, and educational needs of patients and families.

Key responsibilities include:

1. Working with patients and families to understand their concerns and develop personalized plans to address their needs
2. Providing education and support on treatment options, clinical trials, and supportive care services
3. Collaborating with healthcare teams to ensure timely and effective communication and coordination of care
4. Identifying and addressing potential barriers to care, such as financial or social challenges
