As an integral part of the care team, the Advanced Physical Therapist delivers high-quality patient care, adhering to safety protocols and guidelines to ensure optimal patient safety and outcomes.

This position also serves as a mentor and resource for less experienced staff, including other therapists and technicians, and may provide guidance to students and clinical fellows.

Additionally, you will assist in the collection of quality metrics and analytics, contributing to the development of best practices and process improvement initiatives.

Essential Functions:
Team Collaboration:
Contribute to a collaborative work environment by consistently offering assistance and responding positively to requests for help. Foster healthy relationships within the team.

Provide guidance to the interprofessional healthcare team and apply feedback to enhance patient outcomes.

Utilize therapeutic communication techniques, including active listening and education, to build rapport with patients, families, and team members, facilitating collaboration on care plans.

Patient Care:
Conduct thorough evaluations of patients across various diagnoses to determine their functional levels and develop tailored treatment plans.

Implement the physical therapy treatment plan based on evaluation findings, ensuring services are provided to complex patient populations.

Practice in alignment with principles of cultural diversity and inclusion, offering education based on individual learning needs and adapting teaching methods as necessary.

Act as a resource for clinical fellows and nursing staff, sharing best practices and collaborating on treatment modifications and discharge planning.

Quality and Safety:
Regularly assess patients, monitor progress towards treatment goals, and adjust plans as needed to ensure optimal outcomes.

Identify opportunities for process improvement and contribute to meeting departmental quality and safety targets.

Documentation and Compliance:
Maintain accurate and timely documentation of patient education and procedures in patient records, ensuring compliance with all regulatory requirements.

Professional Development:
Engage in ongoing professional development, seeking opportunities to enhance skills and knowledge beyond baseline competencies.

Qualifications:
Graduate of an accredited physical therapy program. Two years of experience as a Physical Therapist or completion of a comparable residency program is preferred.

Skills and Abilities:
Demonstrated clinical skills, strong communication abilities, and a commitment to patient-centered care are essential.
