We are seeking a skilled Pediatric Nurse Case Manager to join our team at The Kidz Club. The successful candidate will be responsible for managing the care of pediatric patients, collaborating with healthcare professionals, and providing holistic care to children and their families.
Key Responsibilities- Care Management: Develop and implement individualized care plans for pediatric patients, ensuring continuity of care and coordination with healthcare providers.
- Client Education: Educate patients and families on immediate care needs, preventative behaviors, and health promotion strategies.
- Collaboration: Work closely with pediatricians, therapists, and other healthcare professionals to ensure comprehensive care and support.
- Case Management: Facilitate referrals to primary providers, specialists, healthcare facilities, and community resources as needed.
- Education: Graduate of an accredited school of nursing with current RN licensure eligible in the state of Florida.
- Experience: At least six months of current experience as a practicing RN with pediatric experience.
- Skills: Basic personal computer skills and comfort with learning electronic charting systems.
- Certifications: Current negative Tuberculosis skin test (PPD) and Current CPR (Basic Life Support) required.
- Competitive Salary: Starting rate of $31.00 per hour, with rate increases based on experience.
