The clinic office team member is responsible for fulfilling assigned tasks, acting as a crucial communication bridge between healthcare providers and patients, their families, referral sources, and other medical offices. This role involves assisting rehabilitation physicians and Advanced Registered Nurse Practitioners (ARNPs) in an outpatient rehabilitation environment. The associate will be accountable for assessing, planning, implementing, and evaluating nursing care for patients in an outpatient clinic setting, catering to adolescent, adult, and geriatric populations, with occasional care for infants or pediatric patients. Furthermore, this associate is expected to embody and uphold the UnityPoint Health Standards of Excellence and the PMR associate-patient relationship policy. Candidates may be licensed as either an LPN or CMA.

Key ResponsibilitiesCore Functions/Responsibilities:

Clinical Excellence and Patient Care
· Efficiently rooms patients utilizing EPIC rooming tabs and adhering to required elements as per regulatory standards.
- Implements strategies to ensure a safe environment for patients and their families.
- Assists healthcare providers during patient examinations and treatments, including EMG, spasticity management, and evaluations.
- Prepares botox as per physician orders for administration.
- Administers medications and nursing treatments within the defined scope of practice.
- Anticipates the needs of the physician for supplies and instruments during examinations.
- Communicates effectively with discharge planners, insurance providers, patients, families, and other relevant parties.
· Completes orientation and annual competencies specific to the hospital and unit.
· Integrates UnityPoint Health's Comfort/C.A.R.E. Professional Practice Model into nursing care.
Care Coordination
· Utilizes the After Visit Summary (AVS) to communicate changes in medication and care plans, ensuring all patients receive a copy.
· Assists in ensuring all consent forms and patient pain contracts are completed according to protocol.
· Schedules tests ordered by physicians, including obtaining necessary pre-authorizations from insurance providers.
· Participates as a phone nurse, managing messages and communicating with providers via EPIC, while following up with patients on care plans.
· Performs medication pre-authorizations.
· Educates patients regarding their treatment plans, tailoring teaching strategies to their specific needs and comprehension levels.
· Prioritizes patient and workplace safety by preventing errors and reporting concerns promptly.
Leadership and Professional Growth
· Aids in the orientation of new team members, serving as a resource and mentor.
· Engages in ongoing professional development and participates in activities that enhance nursing practice through networking and research.
· Utilizes evidence-based learning models.
· Participates in shared governance and performance improvement initiatives within the department and hospital.
Basic Performance Criteria

· Holds current licenses and/or certifications required for the position.

QualificationsCompletion of an accredited LPN or CMA program is required.
