Key Responsibilities- Conduct discharge planning assessments and develop multidisciplinary plans to ensure timely and smooth transitions to post-acute services.
- Collaborate with the healthcare team to ensure appropriate utilization of inpatient resources and identify opportunities for cost-effective outcomes.
- Advocate for patients and families by identifying and addressing cultural, language, and socioeconomic barriers to care transitions.
- Participate in performance improvement teams and programs to enhance the patient experience and promote quality care.
- Graduate of an accredited school of nursing with a minimum of two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing.
- Current licensure as a Registered Nurse in the state of Washington (RN-WA) and BLS certification within three months of hiring.
