Job Summary

We are seeking a highly skilled Registered Dietitian to join our team at Wallingford Nursing & Rehab in Wallingford, PA. As a key member of our interdisciplinary team, you will be responsible for providing expert nutrition care to our residents, developing and implementing personalized nutrition plans, and collaborating with our healthcare team to ensure the highest level of care.

Key Responsibilities:
  • Develop and implement nutrition care plans for residents, taking into account their medical history, dietary needs, and preferences.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to residents, families, and caregivers on nutrition-related topics.
  • Monitor and evaluate the effectiveness of nutrition interventions and make adjustments as needed.
  • Ensure accurate and timely documentation of nutrition assessments, interventions, and outcomes.
Requirements:
  • Registered Dietitian (RD) or licensed dietitian (LD) with a degree in nutrition or a related field.
  • Previous experience in a healthcare setting, preferably in geriatric nutrition.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality care.
