Job Summary

We are seeking a compassionate and skilled Home Health Aide II to join our team at Lee Health. As a Home Health Aide II, you will provide essential care and support to patients in their homes, promoting their independence and well-being.

Key Responsibilities
  • Provide personal care and assistance with daily living activities, such as bathing, grooming, and ambulation.
  • Observe and document patient condition, reporting changes according to the Care Plan.
  • Work collaboratively with the healthcare team to ensure high-quality patient care.
  • Travel to various locations, including Cape Coral, Fort Myers, Lehigh, Estero, and Bonita, to meet patient needs.
  • Cover weekends and team member PTO as needed, and perform other duties as assigned.
Requirements
  • High school diploma or equivalent.
  • Two years of experience as a Home Health Aide.
  • BLS (American Heart Association/Basic Life Support) certification.
  • Valid Florida driver's license and reliable transportation.
  • Excellent interpersonal, customer relations, and communication skills.
