Outreach Coordinator

Job Summary:
The Outreach Coordinator will be responsible for developing and implementing the PACE Marketing and Community Outreach Plan, coordinating outreach activities, and providing marketing presentations to potential referral sources.
Key Responsibilities:
- Develop and implement the PACE Marketing and Community Outreach Plan, following State and Federal guidelines.
- Coordinate outreach activities with PACE leadership and VOANS resources.
- Provide individual and group marketing presentations to potential referral sources.
- Maintain consistent contact with existing and potential referral sources.
- Participate in marketing activities, such as senior fairs and canvassing senior high-rises.
- Plan and coordinate open houses and marketing events to enhance exposure for the Senior CommUnity Care program.
- Attend staff meetings and in-service training as required.
- Follow all Senior CommUnity Care Policies and Procedures and OSHA safety guidelines.
Requirements:
- Experience in marketing, preferably in healthcare or geriatric care.
- Minimum of one year of experience working with the frail or elderly population.
- Valid driver's license and means of transportation within 100 miles or more daily.
- Clear background check.
Preferred Qualifications:
- Degree in Marketing.
- Licensed Practical Nurse (LPN).
- Experience working with interdisciplinary teams.
