About the Role:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to develop and implement patient care plans.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop individualized care plans to address these needs.
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and optimal patient outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
