Responsibilities:
  • Conduct thorough physical and functional assessments of patients to identify their needs and develop personalized plans of care.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Provide evidence-based interventions and education to patients and their caregivers to promote optimal health outcomes.
  • Perform home safety evaluations to identify potential hazards and develop strategies to mitigate risks.
  • Document patient progress and outcomes in a timely and accurate manner.

Qualifications:
  • Current Physical Therapist license in good standing.
  • Current CPR certification with BLS.
  • Valid driver's license and reliable transportation.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.

Physical Requirements:
  • Ability to stand, walk, and sit for extended periods.
  • Ability to lift, position, and transfer patients as needed.
  • Ability to reach, bend, and stoop to perform various tasks.
