Job Overview

We are seeking a skilled Physical Therapist to join our team at a rehabilitation facility in Oakland, CA.

Key Responsibilities:

  • Develop and implement treatment plans for patients in collaboration with referring physicians.
  • Provide hands-on therapy and treatment to patients according to physician-prescribed plans.
  • Assist the nursing department in training Restorative Aides.
  • Supervise Physical Therapy Assistants in patient care, adhering to state regulations.
  • Communicate patient progress and treatment plans with the healthcare team.
  • Participate in patient care conferences and rehabilitation conferences as needed.
  • Document evaluations, progress notes, and recertification in compliance with PT Board regulations.
  • Record treatment changes and provide family/caregiver training for discharge.
  • Assist in discharge planning and recommend durable medical equipment for patient mobility.
  • Foster positive relationships with patients, families, and colleagues.

Qualifications:

  • Valid Physical Therapist License
