As a vital member of our team, you will play a crucial role in providing comprehensive care to patients recovering from various conditions, including stroke, brain injury, and orthopedic injuries.
Key Responsibilities- Conduct initial assessments and reassessments on all patients to ensure accurate and effective care planning.
- Develop and implement individualized care plans, including nutrition and menu management, to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Provide education and training to patients, families, and staff on nutrition and menu management topics.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Bachelor's degree or higher in dietetics or a related field.
- Registration from the Commission on Dietetic Registration.
- State licensure/certification, if required by state.
- Previous experience in an acute care setting, preferred.
