- Collaborate with healthcare teams to provide patient-centered care and support.
- Advocate for patients and their families to resolve crises and ensure access to community resources.
- Develop and implement discharge plans to ensure smooth transitions of care.
- Provide education and support to patients and their families on health-related issues and community resources.
- Masters Level degree in social work required.
- Clinical Social Worker licensure required.
- Excellent interpersonal and communication skills.
- Ability to work effectively in a fast-paced environment.
- Strong knowledge of medical terminology and healthcare systems.
