- Collaborate with Healthcare Teams: Work closely with our interdisciplinary team of healthcare professionals to ensure seamless care coordination and effective communication.
- Conduct Home Visits: Visit patients in their homes to assess their health status, provide medical care, and educate them on self-care and disease management.
- Develop and Implement Care Plans: Create personalized care plans that address patients' physical, emotional, and social needs, and make adjustments as needed.
- Monitor Patient Progress: Regularly assess patients' progress, identify areas for improvement, and make recommendations for further care.
- Education and Licensure: Must be a licensed Nurse Practitioner with a degree in nursing from an accredited institution.
