Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at St. Joseph Residence in Manchester, NH. As a LPN, you will provide high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure exceptional patient outcomes.

Responsibilities:

* Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
* Collaborate with the healthcare team to develop and implement individualized care plans
* Participate in ongoing education and training to stay current with best practices and industry developments
* Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements

Requirements:

* Graduate of an accredited school of nursing
* Current LPN licensure in the State of New Hampshire
* Experience in long-term care or geriatrics preferred
