Position Overview:

- Schedule: Regular Part-Time with potential for full-time expansion
As part of the PACE Program team, you will play a crucial role in providing comprehensive care to our patients, many of whom are elderly or have chronic conditions. The ideal candidate will have a strong focus on clinical excellence, patient satisfaction, and a collaborative approach to healthcare.
Responsibilities:
- Care planning with the PACE Program interdisciplinary team
- Serve as the primary care physician for patients in the program
- Manage and round on PACE participants admitted to long-term care facilities or acute care settings
- Review medical histories and assessments, and implement plans of care with patients and families
- Serve as a collaborative physician for advanced practice providers, if applicable
- Engage in outreach opportunities to expand patient care services
This role is ideal for a physician who is passionate about working with geriatric patients and those with complex healthcare needs. The position also offers the potential for professional growth, with the possibility of expanding into a full-time role as the program continues to grow.
Requirements:
- Doctorate: Required
- Physician License (MD/DO): Required upon hire
- Board Certification in Specialty Discipline: Required upon hire (or eligibility if appropriate)
- Basic Life Support Program (BLS): Required upon hire (American Heart Association)
