Responsibilities:- Provide routine and acute care to patients, including conducting office visits, administering wellness visits, and coordinating care with other providers and specialists.
- Develop and implement personalized care plans, taking into account patients' unique needs and health goals.
- Collaborate with the care team to ensure seamless care coordination and population health leadership.
- Participate in Oak Street Health's innovative care model, focused on quality over volume and centered in our patients' communities.
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification as a Nurse Practitioner through AANP or ANCC.
- Active, non-probationary state Nurse Practitioner license.
- Active DEA license.
- US Work Authorization.
- Competitive salary and quarterly bonus based on quality metrics.
