Job Title: Home Health Aide

Aveanna is seeking a compassionate and skilled Home Health Aide to provide high-quality care to our patients in their homes. As a Home Health Aide, you will be responsible for providing personal care, household assistance, and assistance with daily living activities that improve quality of life for our patients.

Key Responsibilities:
  • Provide services that are ordered by the physician in the plan of care within the scope of practice per state law
  • Perform, assist, evaluate, and document medical care, such as basic first aid, vital sign monitoring, range of motion exercises, ambulation, and transfer assistance
  • Help patients maintain good personal hygiene, such as bathing, toileting, grooming, and dressing
  • Assist with household management, such as home safety and sanitation, infection control, and light housekeeping
  • Thoroughly complete and coordinate all required documentation, such as patient and family records, and report significant changes in patient condition to the RN Nursing Supervisor
Requirements:
  • High school graduate or GED
  • Current CPR certification
  • Valid Driver's License and Acceptable MVR
  • Ongoing Education: A Home Health Aide must receive at least 12 hours of in-service training during each 12-month period
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with aide care and policies and procedures
Additional Requirements:
  • Continuing Education as required by state
  • Georgia: Successful completion of Nurse Aide training and competency evaluation program or Health Care or Personal Care credentialing program recognized and approved by the state
  • Illinois: Appropriate clearing of Illinois Department of Public Health – Health Care Worker Registry, Successful completion of Long-Term Care Assistants and Aides Training Programs Code, or Documentation of current registration from another state
