Key Responsibilities
  • Provide comprehensive nursing care to patients undergoing dialysis treatment in their homes
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes
  • Administer medications and treatments as prescribed, and monitor patients' responses to therapy

  • Provide education and support to patients and families on dialysis treatment options, self-care techniques, and disease management

Requirements
  • Current RN licensure in the state of employment
  • Minimum of 2 years of experience in nephrology nursing, with a focus on home dialysis
  • Strong clinical skills and knowledge of dialysis therapies and treatment options
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex patient data and develop effective care plans
  • Ability to work in a fast-paced environment, with a strong focus on quality and safety
