Salary and Benefits

We offer a competitive salary range of $120,000 - $180,000 per year, based on experience and qualifications. Additionally, you will be eligible for a $20,000 student loan repayment or sign-on bonus for individuals who have not previously participated in this program.

Job Description

The SCC program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. As an APC within SCC, you will provide care to our highest-risk health plan members and work as part of an interdisciplinary team.

Responsibilities:

  • Conduct comprehensive assessments of members' medical needs
  • Manage members through acute medical situations or exacerbations of chronic conditions
  • Ensure accurate and complete documentation, including ICD-10 codes
  • Communicate and collaborate with the interdisciplinary care team
  • Provide patients and caregivers with counseling and education
Required Qualifications

To be successful in this role, you must have:

  • A graduate degree in Nursing (NP) or Physician Assistant studies (PA)
  • An active and unrestricted license in the state where you reside
  • Certification as a Nurse Practitioner or Physician Assistant
  • Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
  • Ability to complete physical requirements of the job
