Job Title: Registered Nurse

We are seeking a highly motivated and compassionate Registered Nurse to join our team as a Home Hemodialysis Registered Nurse (HHD RN). As a key member of our healthcare team, you will be responsible for providing exceptional patient care and education to patients with end-stage renal disease (ESRD).

About the Role:

As a Home Hemodialysis Registered Nurse, you will be working independently to manage your own caseload of patients, providing comprehensive care and education to ensure optimal treatment outcomes. You will be responsible for assessing patient needs, developing individualized care plans, and implementing evidence-based practices to improve patient outcomes.

Responsibilities:
  • Provide patient education on home hemodialysis, including training on dialysis equipment and medication management
  • Conduct regular home visits to monitor patient progress and provide ongoing support
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
  • Document patient progress and outcomes in accordance with regulatory requirements
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 12 months of RN experience
  • Current driver's license, reliable transportation, and vehicle insurance in state of residence
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
  • Basic computer skills and proficiency in MS Word and Outlook
Preferred Qualifications:
  • Previous experience in nephrology nursing or home hemodialysis
  • Training/Teaching/Healthcare Education background
  • Previous case management experience
  • Familiarity with telehealth technologies
