Job Summary:

As a Licensed Practical Nurse (LPN) in our Long Term Care (LTC) department, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents. Under the supervision of the Registered Nurse/Director of Nursing, you will utilize your nursing skills and knowledge to develop and implement individualized care plans that meet the unique needs of each resident.

Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and assisting with daily living activities.
  • Conduct thorough assessments of residents' physical and emotional needs, and develop and implement individualized care plans to address these needs.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
Requirements:
  • Graduate of an accredited school of nursing with a current LPN license.
  • Basic Life Support (BLS) certification.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with residents, families, and healthcare team members.
