Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans to improve their quality of life.

Key Responsibilities
  • Conduct thorough health assessments, including medical history, medication adherence, and social environment
  • Develop and implement personalized care plans to address identified health risks
  • Collaborate with healthcare providers, case managers, and social workers to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, with prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
