Job Title: Director of Social Services
Job Summary:
The Director of Social Services is responsible for leading the Social Services Department at The Willows at Springhurst, a senior living provider. This role involves directing, planning, organizing, and developing the operation of the Social Services Department, ensuring that the medically related emotional and social needs of residents are met and maintained on an individual basis.
Key Responsibilities:
- Plans, develops, and organizes social service programs for the Health Campus.
- Participates in discharge planning, development, and implementation of social care plans and resident assessments.
- Develops a written plan of care for each resident, including preliminary and comprehensive plans.
- Reviews and revises care plans and assessments as necessary, but mainly quarterly.
- Provides information to residents and families on Medicare/Medicaid and other financial assistance programs.
- Reviews and develops a plan of correction for social service deficiencies noted during survey inspections.
- Interviews residents or family members, as necessary, to obtain social history.
- Attends and participates in annual company-required in-service training programs.
- Follows all safety regulations, programs, policies, and procedures.
- Follows resident's rights and responsibilities, including the right of refusal.
- Follows the company's policies and procedures regarding resident complaints and/or grievances.
- Assists in recruiting, screening, selecting, hiring, training, coaching, counseling, and developing a team of qualified and highly motivated individuals.
Qualifications:
- High school diploma or equivalent.
- Bachelor's in Psychology or Licensed Practical Nurse in Kentucky, Michigan, or Ohio.
- Must meet State requirements and regulatory code(s) for licensure or certification.
- Three to five years' experience working in the field of social services in a senior residential care, healthcare, senior living industry, or long-term care environment.
- Exemplary computer skills that include knowledge of the Microsoft Office Suite of products.
Physical Requirements:
- Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours.
- Must be able to lift at least 50lbs.
- Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates.
