Lung Transplant Nurse Coordinator Job Summary

Mass General Brigham is seeking a skilled Lung Transplant Nurse Coordinator to join our team. As a key member of our transplant program, you will be responsible for coordinating the care of patients undergoing lung transplantation.

Key Responsibilities:
  • Coordinate patient care from pre-transplant to post-transplant, ensuring seamless transitions and optimal outcomes.
  • Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to develop and implement individualized care plans.
  • Provide education and support to patients and their families, addressing their physical, emotional, and psychological needs.
  • Manage and coordinate post-transplant follow-up care, including scheduling appointments and communicating with healthcare providers.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in lung transplant care.
Requirements:
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Minimum 2 years of experience in pulmonary, thoracic surgery, or transplant nursing, with a strong background in ambulatory care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Knowledge of community resources and post-acute care providers, with the ability to connect patients with necessary services.
