Work Schedule:
Tuesday - Saturday - 3:30 PM to 11:30 PM; Saturday - 10 AM to 6 PM (twice)
Sunday - Thursday 3:30 PM to 11:30 PM; Sunday 10 AM to 6 PM (twice)

Key Responsibilities

  • Assess physician medication orders for appropriateness regarding drug selection, dosage, potential interactions, and administration routes. Provide recommendations for therapy modifications to physicians and facility staff as necessary.
  • Maintain a comprehensive patient database, including relevant medical history, clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).
  • Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to offer clinical and technical support as required.
  • Document all communications with physicians, nurses, and other healthcare team members, as well as patients and their families, in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and disposal of medications and biologicals.
  • Obtain and maintain all required documentation in compliance with federal and state regulations and organizational policies.
  • Notify the Pharmacy Manager of any medication-related errors.
  • Assist facility staff with procedures related to the safe and effective management of medications.
  • Perform additional tasks as assigned.


Qualifications

  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • Minimum of one year of experience in long-term care.
  • Licenses/Certifications: As mandated by regulatory requirements and business needs.
  • Detail-oriented, goal-driven, team player with strong oral communication skills.
