Position Overview

This is a full-time role offering competitive compensation along with a comprehensive benefits package.

Key Responsibilities

Essential Duties and Responsibilities:

  • Evaluate resident requirements and create, implement, and monitor a detailed care strategy. Collaborate with the resident's healthcare provider to coordinate care effectively. Regularly reassess and modify the care plan as necessary.
  • Partner with a multidisciplinary team to formulate discharge plans tailored to individual residents, facilitating smooth transitions.
  • Serve as a communication bridge among residents, their families, medical professionals, and facility staff.
  • Conduct regular rounds to evaluate nursing care, monitor resident conditions, and ensure a safe and comfortable living environment.
  • Perform audits on nursing staff to verify adherence to established care protocols and policies.
  • Review documentation of deceased residents to ensure compliance with nursing standards during the final hours and confirm that discharge instructions from physicians are properly recorded.
  • Conduct ongoing evaluations of care plans to ensure they are comprehensive and updated by the Charge Nurse as changes arise.

Qualifications

Required Qualifications:

  • Must hold a current, valid nursing license in good standing.
  • At least one (1) year of experience in nursing administration is required. Additional qualifications in rehabilitative or geriatric nursing are advantageous.
  • Strong understanding of relevant state, federal, and local regulations governing long-term care.
  • Excellent communication skills, with a patient and disciplined approach.
  • Proficiency in English, both spoken and written, with adequate communication skills to fulfill job responsibilities.
