Responsibilities:

  • Patient Assessment: Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to address their unique requirements.
  • Care Coordination: Collaborate with healthcare professionals, family members, and caregivers to ensure comprehensive care and support.
  • Documentation: Accurately record patient progress, reactions, and symptoms, maintaining detailed records in accordance with agency, state, and federal guidelines.
  • Education and Support: Provide patients and their families with education and support to promote healthy behaviors, disease management, and overall well-being.

Requirements:

  • Education and Licensure: Graduation from an accredited nursing school and a current, valid RN license registered in the state of Texas.
