Job Summary

We are seeking a highly skilled Assessment Clinician to join our team at UHS. As a key member of our clinical staff, you will be responsible for conducting thorough assessments of patient admissions and providing critical information to individuals, agencies, and professional staff.

Key Responsibilities:

  • Conduct comprehensive assessments of patient admissions to identify treatment needs and develop individualized care plans.
  • Collaborate with multidisciplinary teams to ensure seamless transitions of care and effective communication.
  • Provide education and support to patients, families, and caregivers to promote optimal health outcomes.
  • Participate in quality improvement initiatives to enhance clinical practices and patient satisfaction.
  • Develop and maintain relationships with referral sources to ensure a steady flow of admissions and optimize treatment resources.

Requirements:

  • Master's degree in a clinical area, such as counseling or social work, from an accredited university.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Physical stamina to perform job duties, including standing, walking, and lifting up to 20 pounds.
