Job Title: Private Duty Nurse LPN

Aveanna Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. As a Private Duty Nurse LPN, you will be responsible for delivering high-quality patient care, documenting patient progress, and collaborating with healthcare teams to ensure optimal patient outcomes.

Job Summary:

The Licensed Practical Nurse (LPN) is a vital member of our home health care team, providing personalized care to patients in their homes. As an LPN, you will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Deliver high-quality patient care, including administering medications, wound care, and vital sign monitoring.
  • Document patient progress, including medical history, treatment plans, and patient outcomes.
  • Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive patient care.
  • Provide education and support to patients and families on disease management, self-care techniques, and prevention strategies.
  • Participate in ongoing education and training to maintain knowledge of best practices in home health care.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements:
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Must be able to sit and climb stairs.
  • Must have visual and hearing acuity.
  • Must have strong sense of smell and touch.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
Environment:
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
