Job Summary

We are seeking a skilled RN Case Manager to join our team at Jepsen Investments, Inc. in a hybrid position. As a RN Case Manager, you will coordinate patient care activities, ensuring adherence to clinical policies, protocols, and regulations. Our client, a network of Federally Qualified Health Centers (FQHC) on the south side of Chicago, is looking for a temp-to-hire Nurse Case Manager with experience handling high-risk patients, including OB perinatal needs.

Key Responsibilities
  • Coordinate service with patient care units, including interdisciplinary teams
  • Establish goals and strategies for meeting care coordination needs across settings
  • Consider factors related to safety, effectiveness, and fiscal accountability in planning and delivering care
  • Initiate the development of a care plan for each patient, including performing a risk assessment and assigning risk to 100% of caseload
  • Engage the interdisciplinary care team to manage quality care and services
  • Track appointments and monitor no shows or non-compliance to re-establish patients for continuity of care
  • Oversee care for management of multiple disease processes for diagnosed patients
  • Provide follow-up on abnormal labs, communicate treatment plans, and provide appropriate patient education
  • Liaison with Labor and Delivery departments at affiliated hospitals or referral institutions for continuity of care and discharge planning
  • Prepare patients for delivery at assigned hospitals by assisting to set up hospital tours and establishing pre-birth visits
  • Provide post-delivery services, including newborn support, family planning options, and appropriate inter-conceptual spacing education
Requirements
  • Minimum of a bachelor's degree in Nursing required
  • Must have a valid license in the state of IL
  • A minimum of two (2) to five (5) years of acute nursing experience preferred
  • Previous experience in case management of obstetrical patients highly desirable
  • Good communication, inter-personal, and organizational skills
  • Flexible and energetic with great Customer Service Skills
  • Current CPR Certification
  • Bilingual ability is a plus
  • Good Computer skills are required due to software navigation and documentation required
