Job Summary

We are seeking a highly skilled Occupational Therapist to join our team at Skilled Rehab Facility. As an Occupational Therapist, you will play a vital role in providing quality rehabilitation care to our residents, helping them to achieve their maximum potential and regain their independence.

Key Responsibilities
  • Assess the functional levels and capabilities of residents, identifying areas for improvement and developing personalized treatment plans.
  • Conduct ongoing assessments of resident muscle strength, range of motion, coordination, and balance, making recommendations for therapy treatments.
  • Provide physical therapy treatments, following the resident's plan of care and making adjustments as needed.
  • Develop measurable and realistic goals for residents, based on their care plan and progress.
  • Participate in discharge planning, ensuring a smooth transition for residents back to their homes or communities.
  • Document treatment progress in resident charts, accurately and in a timely manner.
  • Maintain a positive working relationship with residents, families, and colleagues, providing excellent customer service and communication.
  • Supervise Certified Occupational Therapy Assistants (COTAs) and rehabilitation aides, ensuring quality rehab care services are provided.
  • Complete required facility forms and documents, ensuring compliance with state and federal regulations.
Requirements
  • Graduate of an accredited Occupational Therapy program (BS or MS-Occupational Therapy).
  • Current OT license for the state of practice in good standing.
  • One year of experience in a healthcare setting, with long-term care experience preferred.
  • Ability to read, write, and speak the English language, communicate resident condition to others, and make independent decisions as needed.
  • Common working knowledge of all rules, laws, and regulations as defined by the state as it relates to physical therapy.
  • Ability to implement Occupational Therapy programs, goals, and objectives needed in the rehabilitation department.
  • Ability to move frequently throughout the workday, lift 40 lbs floor to waist, lift 10 lbs waist to shoulder, lift and carry 40 lbs, and push/pull 40 lbs.
Working Conditions
  • Works specifically in the rehabilitation department, resident rooms, etc.
  • Bends, lifts, sits, stands, and moves frequently throughout the shift.
  • Deals with frequent interruptions and possibly upset residents and family members.
  • Works regularly scheduled shifts with possible weekends, holidays, and other hours based on resident needs.
  • Primary responsibility is care of residents but also deals with visitors, government personnel, etc.

  • Professionalism: Reports to work on time and as scheduled, keeps open communication with direct supervisors, promotes teamwork, and displays a positive, caring attitude.
  • Resident Rights: Treats all resident care information with confidentiality, continuously monitors assigned areas to ensure residents are treated fairly and with dignity and respect at all times.
  • Safety Awareness: Follows safety program guidelines, immediately reports accidents and incidents to supervisors, identifies unsafe working areas and promptly reports to supervisors.
  • Organizational and Communication Skills: Handles multiple residents simultaneously, completes assignments in an organized fashion by the end of scheduled shifts, operates with flexibility, and shows attention to detail when recording information in resident charts.
  • Customer Service: Treats all residents, families, co-workers, and supervisors with dignity and respect, takes initiative to exceed customer expectations, and demonstrates a positive attitude and willingness to help to all visitors and residents.
