About the Role

We are seeking an experienced RN to join our dynamic team. In this role, you will be responsible for providing high-quality patient care, collaborating with multidisciplinary teams, and ensuring seamless communication between clients and providers.

Key Responsibilities:

  • Evaluating Patient Needs: Perform lab tests, obtain vital signs, and assess physical status to inform treatment decisions.
  • Documentation and Communication: Maintain accurate records, prepare reports for providers, and facilitate referrals to other healthcare professionals and facilities.
  • Liaison and Collaboration: Serve as a liaison between clients and providers, communicate effectively with agencies and providers, and represent PATH's philosophy of compassion and kindness.
  • Client Support: Assist clients with medication administration, maintain ethical standards, and provide emotional support during challenging times.

Qualifications:

  • Licensure and Experience: Unrestricted State nursing license RN, 2 years of nursing experience preferred.
  • Skillset: Excellent verbal and written communication skills, ability to work under stressful conditions and irregular hours, competency in Electronic Health Record (EHR) program and telehealth services.
  • Background Checks: Pass federal and state background checks.

Benefits:

  • Competitive Salary: $65,000 - $85,000 per year, depending on experience.
