Responsibilities
  • Assist with health and safety procedures to ensure a safe and healthy environment for individuals supported.
  • Support individuals in developing their skills and abilities to live independently and participate in their communities.
  • Provide emotional support and companionship to individuals, promoting their overall well-being and quality of life.
  • Assist with daily living tasks, such as personal care, meal preparation, and household chores.
  • Document and report on individual progress and outcomes, ensuring accurate and timely information for care planning and decision-making.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans, promoting person-centered and holistic approaches to care.
Requirements
  • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
  • At least 18 years of age and valid driver's license (if required for transportation).
  • Ability to lift 30 pounds and perform physical tasks as required by the position.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and interdisciplinary teams.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
