Job SummaryThe Speech/Language Pathologist will provide high-quality evaluation, treatment, and supervision services to patients with communication, cognitive, and swallowing difficulties. This role requires strong interpersonal skills, emotional intelligence, and the ability to exercise independent judgment and discretion.
Key Responsibilities- Evaluate patients for rehabilitation services within 24 to 48 hours of physician's orders.
- Develop effective treatment plans that address patient needs and goals.
- Treat patients according to the treatment plan of care, ensuring a high level of patient satisfaction and outcomes.
- Communicate effectively with physicians, clinical managers, and other healthcare team members to ensure seamless care coordination.
- Participate in care conference meetings and interdisciplinary team meetings to share knowledge and best practices.
- Provide in-service training programs for facility staff on restorative and functional maintenance programs.
- Record evaluations, treatment notes, progress notes, and discharge summaries in a timely and accurate manner.
- Meet or exceed minimum productivity standards established by Concept Rehab.
- Recommend equipment and resources to improve clinical services and patient outcomes.
- Act as a clinical supervisor for affiliating Speech/Language Pathology graduate clinicians.
- Comply with state-specific Speech Pathology licensure standards.
- Master's Degree in Speech-Language Pathology from an accredited institution.
- Certification by the American Speech-Language-Hearing Association (ASHA).
- Licensure in the state(s) where services are provided to practice as a Speech/Language Pathologist.
