Job SummaryWe are seeking a skilled Patient Care Coordinator or Medical Case Manager to join our team. As a key member of our staff, you will be responsible for coordinating patient care, managing transitions, and communicating with healthcare providers to ensure seamless care coordination.
Responsibilities- Assess and develop care plans for high-risk patients
- Coordinate patient transfers between facilities
- Communicate with healthcare providers to ensure continuity of care
- Document patient information and progress
- Identify and address psychosocial needs of patients and families
- Facilitate referrals to community resources
- Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program
- Current CA RN or LVN license, current CPR certification, valid CA Driver's license
- 3 years acute care or case management experience preferred
- 2-3 years of utilization or HMO experience preferred
- Typing 40 words per minute with accuracy
- Knowledge of computers, faxes, printers, and all other office equipment
- Knowledgeable in MS Office Programs (i.e., Word, Excel, Outlook, Access, and PowerPoint)
The estimated salary for this position is $38-$50 per hour, depending on experience and qualifications. In addition to competitive compensation, we offer a comprehensive benefits package, including medical, dental, vision, and life insurance, as well as 401(k) eligibility and paid time off.
