About the Position:


During a visit that may last up to one hour, Matrix providers assess and evaluate a member's current health status, medical history, medication compliance, social circumstances, and other risk factors.

This offers unparalleled insight into a member's overall health and wellness that can be challenging to capture during standard office visits.

The Matrix Comprehensive Health Assessment enhances care quality and allows us to potentially address multiple care gaps in a single encounter.

Responsibilities:

Matrix Provider – What to Expect:
Conduct assessments for adults and geriatrics, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in accordance with Matrix protocols.

Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and any relevant ancillary medical staff as needed.

Engage with Primary Care Physicians (PCP) on patient education and provide follow-up care.

Deliver services across various settings, including home visits, skilled nursing facility visits, and telehealth consultations.

Clinical Nurse Specialist or Physician Associate - PRN/Part Time - Home Health Evaluations

Type:
PRN/Part Time - Flexible - Clinical Nurse Specialist/Physician Associate

PAY:
$115.00 per completed assessment

Location:
Home, Skilled Nursing Facilities, Telehealth, and other community environments

Hours:
Days, Evenings, Nights, Weekends, Flexible Scheduling

Benefits:
PRN/Part Time providers are non-benefited except for 401(k) and limited voluntary benefits.

Mileage reimbursement plan.

Qualifications:

Must Haves:
Master's Degree required OR equivalent experience and successful completion of NP licensure.

Master's Degree required OR equivalent experience and successful completion of PA licensure.

Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.

Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.

Current BLS, ACLS, or CPR certification.

A minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant. Ability to travel as needed.

Strong computer skills and familiarity with EMR software.

Excellent verbal and written communication skills with patients, clients, and colleagues.

A positive and outgoing personality, with the ability to work collaboratively and adapt to frequent changes.

Travel Requirements:
Ability to travel, possess a valid state driver's license, and maintain proof of adequate automobile insurance coverage.

Travel is required to meet patients in their homes.
