MEDSTAR HEALTH is seeking a PRN Social Worker LCSW-C to join our team at MedStar Harbor Hospital. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support.
Key Responsibilities:
- Collaborate with healthcare team members to identify patient needs and develop comprehensive care plans.
- Conduct thorough psychosocial assessments and provide timely referrals to community agencies as needed.
- Coordinate the completion of necessary forms and documents in a timely and accurate manner.
- Develop and maintain knowledge of community resources and referral requirements to support patient needs.
- Document patient activities and maintain statistical data as required.
- Facilitate effective communication between patients, families, and healthcare teams.
- Assess and communicate patient social, cultural, emotional, and economic concerns to medical staff and other healthcare providers as needed.
- Identify opportunities for performance improvement and contribute to solution-oriented approaches.
- Organize workload and prioritize tasks based on patient needs and policy requirements.
- Orient and supervise staff and students as assigned.
- Participate in multidisciplinary care coordination rounds and collaborate with internal and external healthcare providers to develop comprehensive discharge plans.
- Provide clinical assessment, evaluation, and disposition of individuals presenting with behavioral health concerns or illness.
Requirements:
- Master's degree in social work from a school accredited by the Council on Social Work Education.
- 1-2 years' experience in social work, preferably in a hospital setting.
