The Registered Nurse (RN) provides high-quality patient care in a home and community environment. This role requires assessing and monitoring patients' physical condition, supporting patient/caregiver education, and providing treatments and procedures based on a physician-ordered plan of care.
Responsibilities- Provide comprehensive care to patients, addressing their physiological, psychological, socioeconomic, developmental, educational, and safety needs.
- Assess patients' physiological, cognitive/mental, developmental, emotional, and social status to develop an effective care plan.
- Implement treatments and procedures based on a physician-ordered plan of care, ensuring patient safety and well-being.
- Deliver patient and/or caregiver education as needed, promoting health literacy and self-management skills.
- Collaborate with the patient care team to provide input on patient plan of care and progress toward goals.
- Maintain a clean and safe working/patient care environment, adhering to infection control and safety protocols.
- Document all treatments/care provided and patient responses to treatments, ensuring accurate and timely reporting.
- Communicate changes in patient condition to the physician, Case Coordinator, and/or Clinical Supervisor as indicated.
- Provide medical clinic nursing coverage to youths at the Summit County Juvenile Detention Center (JDC) as assigned.
- Perform other duties as required, ensuring flexibility and adaptability in a dynamic healthcare environment.
- Graduation from an approved accredited Registered Nursing program is required.
- Current Ohio licensure as a Registered Nurse is required.
- Current CPR/BLS certification through the American Heart Association is required.
- Experience in home health or an acute care setting is preferred.
- Experience in Pediatrics and/or working with individuals with developmental delays is preferred.
- Experience in managing technology-dependent and/or medically complex patients is preferred.
- Proficiency in MS Office (Outlook, Excel, Word) or similar software is required.
