Job Summary

We are seeking a highly skilled and experienced Registered Physical Therapist to join our team in the West Side communities and surrounding areas. As a Physical Therapist with Discovery At Home, you will provide skilled Physical Therapy services on an intermittent basis to clients in their homes, in accordance with physician orders.

Responsibilities:
  • Conduct comprehensive assessments of clients' needs, including OASIS assessments.
  • Develop and implement treatment plans in consultation with physicians and revise as necessary.
  • Assist in the development and implementation of interdisciplinary client care plans.
  • Treat clients to relieve pain, develop or restore functional abilities, and maintain maximum performance using physical means.
  • Consult and educate staff on established plans.
  • Teach, supervise, and counsel clients and their families on physical therapy programs.
  • Provide equipment and arrange for out-patient services as needed.
  • Record evaluation data, treatments, and clients' responses to therapy intervention.
  • Prepare assessments and periodic evaluations and summaries.
  • Supervise Physical Therapy Assistants and paraprofessionals.
  • Communicate with physicians and other team members.
Qualifications:
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of institutional experience.
  • At least 2 years of home care experience and at least one year supervisory experience.
