A RN Case Manager opportunity is available at SSM Health Saint Louis University Hospital. This role will work with a collaborative team to support in-patient units and specialties. The ideal candidate will have excellent communication skills and be able to work effectively with Social Workers for care coordination and discharge planning.Responsibilities and Requirements
As a RN Case Manager, you will assess, develop, plan, implement, and evaluate patients' discharge planning needs to next level of care in collaboration with an interdisciplinary team. Key responsibilities include ensuring compliance with Condition of Participation (CoP) discharge planning, identifying and escalating gaps/barriers related to in-patient and observation length of stay, assessing patients at high risk for readmission, and developing plans to prevent readmissions. Additionally, you may be responsible for utilization management process, ED case management process, admission process, and acting as a case manager liaison with post-acute providers.Requirements
To be successful in this role, you will need to have a graduate degree in nursing or education equivalency for licensing, three years' acute care experience, and a strong understanding of the ANA Code of Ethics for Nurses. You must also be able to work in a constant state of alertness and safe manner, perform other duties as assigned, and meet the physical requirements of the job.Benefits
