Montefiore Medical Center is seeking a skilled Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Act as a liaison between patients, families, and physicians to ensure seamless care coordination.
  • Provide regular updates on patient condition to families and physicians.
  • Ensure all in-home medical equipment is in working order and properly maintained.
  • Document all details of patient's condition, progression, and regression activity.
  • Oversee required physical therapy or rehabilitation exercises to promote patient recovery.
  • Contact emergency services when patients require hospitalization.
  • Possess knowledge of Home Care reimbursement processes and OASIS documentation.
  • Have reliable transportation for home visits and be able to walk up/down six flights of stairs as needed.
Requirements
  • Minimum of one year hospital Med Surg experience.
  • One year of Home Health experience or Certified Home Care (CHHA) experience preferred.
  • NYS RN License and Registration in good standing.
  • Bachelor's degree.
  • Infection Control certificate.
  • Valid Driver License, Registration, and Automobile Insurance.
  • Knowledge of a second language a plus.
