Job Summary

Huntsville Memorial Hospital is seeking a skilled Registered Nurse to fill the role of Patient Navigator. This part-time position will work weekends, providing exceptional patient care and navigation services to our community.

Key Responsibilities
  • Assist patients and families in understanding their diagnosis, treatment options, and available resources, including educating eligible patients about strategies to improve quality of life and reduce hospital admissions.
  • Facilitate post-hospital follow-up appointments with physicians, dietitians, social workers, and other healthcare providers, ensuring seamless care coordination.
  • Collaborate with community agencies to identify potential resources for resolving clients' health, psychosocial, or financial problems that impact their healthcare and hospitalizations.
  • Serve as a vital link between patients and all other care providers, ensuring effective communication and care delivery.
  • Coordinate multidisciplinary planning conferences as needed, develop patient summaries, and document recommendations using standardized care protocols in accordance with nationally recognized care guidelines.
  • Develop patient education programs and tools to promote health literacy and self-management.
  • Follow patients with high-risk readmission diagnoses and/or frequent hospital admissions throughout the care continuum, collaborating with patient care management resources.
  • Identify patients with no primary care physician and provide options based on their preference and payor source.
  • High-risk patient populations will be contacted within hours post-hospitalization to address barriers or concerns related to their care and determine if intervention is needed.
  • Patients with frequent hospitalizations will receive additional follow-up phone calls to remind them of their physician's appointment and then after the appointment to determine the outcome.
  • The designated caregiver of high-risk patients will be contacted with the patient's authorization to update them on care needs and challenges that may be addressed to reduce hospitalization and readmission.
  • Collect data, track outcomes, and support strategic planning to reduce hospital readmissions.
  • Adhere to productivity benchmarks and maintain accurate records.
  • Establish and maintain working relationships with key customers, including physicians, office staff, diagnostic staff, nurses, and other healthcare professionals.
  • Act as a client advocate in securing education, healthcare, counseling, transportation, medication, and other necessary services.
  • Assess clients dynamically, develop case management plans, and work with clients to meet plan goals.
  • Accurately identify and prioritize at-risk community populations.
  • Demonstrate the ability to use teaching, learning, and counseling skills.
  • Assume leadership roles to support the patient population served.
  • Serve as a preceptor/evaluator for various disciplines.
  • Coordinate primary and secondary prevention opportunities to promote early identification and intervention for various populations.
  • Lead and/or participate in interdisciplinary team meetings.
  • Utilize advanced assessment skills to work autonomously with community agencies, hospitals, healthcare providers, and social service agencies.
  • Abide by Huntsville Memorial Hospital's Legal Compliance Code of Conduct.
  • Maintain patient confidentiality and handle PHI appropriately.
  • Maintain a safe work environment and report safety concerns accordingly.
  • Perform all other related duties as assigned.
Requirements
  • Education: Graduate of a school of professional nursing.
  • Experience: RN with two years of Acute Care Hospital Case Management/Patient Navigation experience preferred. Will consider LVN with Acute Care Case Management/Care Coordination/Patient Education experience.
  • Licensure/Certification: Current licensure as a Registered Nurse in the state of Texas, or immediately eligible.
  • Required Skills: Strong analytical, data management, organizational, and time management skills. Knowledge of teaching/learning theories and skills in developing and providing individual and group education/training. Knowledge of community resources. Skilled in assessing clients with complex social, mental, and health problems. Skilled in creating interpersonal relationships, interacting with all levels of education, social differences, and various ethnicities and cultures.
Work Environment

This part-time position will work weekends, with frequent sitting, standing, walking, and reaching. Occasional lifting, carrying, bending, and squatting may be required. Visual and hearing acuity are necessary. Work is mostly inside, with good ventilation and comfortable temperature, as well as outside in the community and may require some driving.

Possible exposure to toxic/caustic chemicals or detergents, communicable diseases, and blood-borne pathogens.
