About the Role:

Institute on Aging is seeking a highly skilled and compassionate Care Manager II to join our team. As a Care Manager II, you will play a critical role in ensuring the health and well-being of our clients by assessing their needs, developing and implementing personalized care plans, and coordinating services to support their independence and quality of life.

Responsibilities:

  • Conduct comprehensive assessments of clients' physical, emotional, and social needs
  • Develop and implement individualized care plans that address clients' unique needs and goals
  • Coordinate services and interventions to support clients' independence and quality of life
  • Provide education and support to clients and their families on health and wellness topics
  • Collaborate with healthcare providers and community resources to ensure seamless care coordination
  • Document all case management activities and client interactions in a timely and accurate manner

Requirements:

  • M.S.W. (Masters in Social Work) degree required; LCSW preferred
  • One year of experience working with disabled adults and/or older adults required
  • Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults
  • Exceptional communication and presentation skills relating to functionally impaired adults and older adults, their support systems, and teams of health professionals
