Overview: We are seeking a skilled Memory Support LPN to provide direct nursing care to residents, promoting their physical and emotional well-being in a supportive environment.
- Supervise and coordinate activities performed by Nursing Assistants, ensuring a safe and supportive environment.
- Responsible for admission, transfer, and discharge of residents, maintaining accurate records and communicating with healthcare professionals.
- Keep physician and/or other healthcare professionals informed of changes in resident conditions, ensuring timely interventions.
- Attend regular in-service education sessions to stay up-to-date on best practices and industry developments.
Required Skills and Qualifications:
- Graduate of an accredited LPN program.
- LPN license for the state in which practicing.
- Recent clinical experience, education, and specialty skills specific to geriatrics.
- Flexible schedule, including availability to work evenings, weekends, and holidays as needed.
- Passion for working with seniors and commitment to delivering exceptional care.
