Your RoleWe are seeking a skilled Care Coordinator to join our team of compassionate professionals. As a Geriatric Care Coordinator, you will play a vital role in ensuring that our participants receive personalized care and support to maintain their independence and dignity.
Key Responsibilities- Interdisciplinary Team Collaboration: Participate in initial assessments, care planning, and periodic re-assessments to ensure seamless coordination of services.
- Care Plan Development: Develop person-centered and comprehensive care plans that address the unique needs and goals of each participant.
- Communication and Advocacy: Effectively communicate participant changes to the team, engage in IDT meetings, and advocate for their needs.
- Discharge Planning: Coordinate discharge planning and admissions to long-term care facilities, ensuring a smooth transition for participants.
- Record Keeping: Maintain accurate records, including progress notes, care plans, and periodic reassessments.
- Community Resource Linkage: Connect participants with entitlements through public agencies and refer them to community resources as needed.
- Masters Degree in Social Work (MSW): Hold a degree from a CSWE-accredited program.
- Licensed Master Social Worker (LMSW): Be actively licensed or have a Limited Licensed Master Social Worker (LLMSW) designation.
- Experience: Prefer one year of experience working with the frail elderly or long-term care population.
