Travel Nurse Case Manager Job Description

We are seeking a skilled RN Case Manager to join our team at Travel Nurse Across America. As a RN Case Manager, you will work with a highly skilled team of professionals to advocate for patients, evaluate, plan, provide resources, and facilitate communication with family members. Your primary goal will be to coordinate comprehensive short and long-term care for patients, aiming to decrease hospital stay lengths and reduce readmission rates to generate cost-effective outcomes.

Key Responsibilities:
  • Evaluate and assess new patients to develop effective care plans.
  • Create and update comprehensive short and long-term patient care plans to ensure optimal outcomes.
  • Educate families and patients about available resources and healthcare options to empower informed decision-making.
  • Collaborate with other RNs, doctors, and social workers to provide seamless care coordination.
  • Properly document and submit medical records to ensure accurate and timely communication.
  • Communicate with insurance companies to collaborate or appeal decisions, promoting patient-centered care.
  • Analyze results, monitor variances, identify trends, and recommend actions to management to improve financial status.

Requirements:
  • BLS certification
  • 2 years of recent case management experience

Please note that additional certifications may be required before beginning an assignment.
